Having watched Cold Case since the very beginning and seeing all the episodes along the way, I have to say there's only ever been one episode that I've found that I disliked enough to avoid ever watching it again on repeat and that was from season 4.
My only vote in this category goes to:

Ahh... And now, my "Least Favorite Category": Going over episodes I didn't like. Oh well, let's get over this....
"Blood on the Tracks" -- Not a single likeable character in the story.
"A Dollar, A Dream" -- The larger issue of Homelessness was overshadowed by a formulaic and unrealistic story of a single mom and her two daughters. The story could NEVER happen in Philadelphia. Living on the streets here is simply too dangerous for a white suburbanite mom with two little girls.
"The War at Home" -- The characters in the case were awfully stereotyped and lacked any kind of depth. I felt very strongly that the writer had no understanding of working-class neighborhoods and the family values of people who live there. The episode was unusually lacking on meaningful and relevant symbolisms too.
